Privacy Notice

We are committed to safeguarding the privacy of all patients. This document has been devised to reflect the new Data Protection Act 2018 which includes the GDPR (General Data Protection Regulation).

Basis For Collecting Data

  1. We collect personal information about your health in order to provide you with the best possible treatment. Your request for treatment and our agreement to provide that care constitutes a ‘Contract’. You can refuse to provide the information but if you chose that option we would not be able to provide you with care. The data collected: name, address, telephone numbers, email address, date of birth, health information including medical history, diagnosis and treatment data.
  2. We have a ‘Legitimate Interest’ in collecting this information because without it we couldn’t do our job effectively and safely.
  3.  It is important that we can contact you in order to confirm your appointments or to update you on matters related to your medical care, which again is considered a ‘Legitimate Interest.’
  4.  Provided we have your consent we may send you health information in the form of articles or letters.

Records

  1. We have a legal obligation to retain your records for 8 years after your most recent appointment (or age 25 if this is longer) but after this period you can ask us to delete your records if you wish.
  2.  Your records are stored on paper, which are kept in a locked filing cabinet and the office is always locked and alarmed out of hours.
  3. No patient medical files (consultation, examination, treatments) are computerised.

Access

  1. We will never share your data with anyone who does not need access without your written consent.
  2. The only person having access to your medical records (medical letters & reports, consultation, examination and treatment notes) is the chiropractor.
  3. In carrying out certain tasks for the clinic, outside agencies may have access to your contact data only such as: administrative, legal, financial, or computer services. They will NOT have access to any medical information as outlined above and will understand that the information must be treated with confidence.
  4. On occasions, your data and health information may be used when contacting other health professionals or associations with respect to your care including GP’s, Hospitals and Health Insurance Companies. Your consent for this will be obtained at the time unless we are required to do so by law.

Your Rights

  • You have the right to see what personal data of yours we hold, and you can also ask us to correct any factual errors. Provided the legal minimum period has elapsed, you can also ask us to erase your records. Please provide your name, address, telephone, email address and details of the information you require. Your identity will need to be verified so we may ask for a copy of ID.

Concerns

  • We want you to be confident that we are treating your personal data responsibly. If you feel that we are mishandling your personal data in some way, contact the data controller/protection officer and the ICO via www.ico.org.uk if dissatisfied with our response.
